Morningstar health, LLC providers do not accept insurance and operate on a fee-for-service model.

All fees are due at the time of service. 

There are many benefits to the direct pay model, including increased confidentiality over your private information, reduced emphasis on diagnoses, and greater flexibility in treatment duration and frequency. 

Phone Calls: Lasting greater than 10 minutes will be billed at a prorated rate of $150/hour for psychiatric provider. This includes phone calls with the patient, other providers, insurance companies (including for prior authorization medication requests), family members, other treatment facilities, etc. Fees will be charged to the patient’s credit card kept on file.

Paperwork/Letters: Forms, medication prior-authorizations, letters, or other paperwork will be assessed a base fee of $35 and any time above 15 minutes will be billed at a prorated rate of $250/hour for psychiatric provider (except as otherwise set forth below under Legal-Related Services). The base fee for forms related to disability, leave, retirement or accommodation requests is $50. Fees will be charged to the patient’s credit card kept on file. Paperwork can take up to 10 business days to complete. Short term disability/leave paperwork is only completed if there is an established/ongoing treatment relationship with a patient.

Legal- Related Services: All services related to legal proceedings will be assessed at $250 per hour (including but not limited to court testimony, depositions, legal conferences, preparation of reports/letters, travel/waiting time, and records/chart review). Unless otherwise arranged, an estimated fee is due 5 days in advance of a court or deposition appearance, and the balance (if any) is due within 48 hours after. If the court hearing/deposition is canceled for any reason

less than 5 business days prior to the date, the full fee is charged (for time spent preparing plus two hours). Fees will be charged to the patient’s credit card kept on file.

Missed Appointment /Late Cancellation Fee: A minimum of 48 business hours’ notice is required for cancellation of all appointments. Late cancellations or no-shows will result in the full appointment fee being charged (this includes initial appointments). The credit card on file will be charged. Arriving 15+ minutes late for your appointment is considered a no show. Cancellation fees will be waived in the case of a medical emergency. More than three missed appointments within a year may result in discharge from the practice.  

You are responsible for remembering your appointment date and time. Automated reminders may be sent but should not be solely relied upon for scheduling.  If a working email/phone number is on file and you elect to receive reminders, automated reminders will be sent    

Urgent Prescription Refill:  A $25 fee will be charged for 

1.Refill requests made outside of business hours (Monday-Sunday). 2.Refill requests made when a patient has not scheduled/attended a follow-up appointment and needs medication to get through until the next available appointment.
